B12 supplements: sublingual, injection, or pill, and whether form matters for absorption

Vitamin B12 supplements are sold in three main forms: standard swallowed pills, sublingual tablets that dissolve under the tongue, and intramuscular injections given by a doctor or nurse. Marketing around sublingual and injectable forms often implies they bypass the gut and work better as a result. A 2025 meta-analysis pooling 16 studies and more than 6,000 participants tested that claim directly.[1]

The numbers look different route to route, but the statistical comparison between them showed no statistically reliable difference, with a p-value of 0.270 for serum B12 and 0.485 for homocysteine change.[1] In plain terms, the differences shown in the table are small enough that they could plausibly be due to chance rather than a real advantage for any one form. The researchers also found no dose-response relationship: a higher-dose pill did not reliably produce a bigger effect. That suggests how well a form is absorbed matters more than how much of it you swallow.

This matters most for a specific group: people with pernicious anemia or other conditions that prevent the stomach from making intrinsic factor, a protein B12 needs to be absorbed through the small intestine (see why your gut might stop absorbing vitamin B12 for how this happens). For most other people, standard oral B12 relies on a passive absorption pathway that still works even without intrinsic factor, just less efficiently, part of why high-dose oral pills can still raise blood levels effectively.

What to do with this

For most people without a diagnosed absorption disorder, a standard oral B12 pill, typically 500 to 1,000 micrograms daily for correcting a deficiency, performed statistically the same as sublingual tablets and injections in this analysis. Save injections for cases your doctor has specifically flagged, such as pernicious anemia, extensive small intestine surgery, or a documented malabsorption issue, where the evidence for a real difference is stronger. If a sublingual product costs meaningfully more than a plain pill, that premium isn't backed by this data. The distinction between digestion and absorption matters for other nutrients too, and stomach-acid support supplements like betaine HCl raise a similar question about who needs them.
