Smoking's damage to your lungs and heart gets most of the attention, but the same smoke passes through your digestive tract on its way there, and your gut bacteria feel it directly.
What smoking does to the makeup of your gut bacteria
A systematic review pulling together 13 studies on cigarette smoking and gut bacteria found a consistent pattern: most studies showed smokers carrying less diverse gut bacteria than non-smokers, along with lower variability in which species showed up from person to person.[1] One bacterial genus, Prevotella, showed up consistently higher in both current and former smokers, though researchers are still working out exactly what that shift means for health.
How smoke reaches bacteria that never touch your lungs
Nicotine and other tobacco compounds don't stay confined to your airways. They get swallowed along with saliva and travel through your entire digestive tract, reaching bacteria far from your lungs.[2] Researchers have also found that smoke exposure changes how your gut bacteria metabolize certain toxins, which may partly explain why smoking raises cancer risk in organs beyond the lungs. A systematic review of 13 separate studies found this lower gut bacteria diversity in smokers to be a consistent finding despite differences in study design and population.[1]
The cancer connection researchers are still mapping
A 2025 study found that smoke-altered gut bacteria can influence how the immune system responds to tumors, using fecal transplants from smoke-exposed mice to show the effect could be passed along through gut bacteria alone.[3] This doesn't mean gut bacteria cause cancer on their own. It means the well-established link between smoking and cancer risk may run partly through the gut, alongside the direct damage smoke does to cells it touches. What happens when you quit
Research on smoking cessation has found some encouraging signals. In one study of people with coronary artery disease, smoking was linked to lower levels of certain beneficial bacteria and higher levels of others tied to inflammation, and those shifts moved back toward a healthier pattern after quitting.[4] Researchers have also started looking at whether gut bacteria changes play a role in nicotine withdrawal symptoms themselves, an early but interesting area of study given how much withdrawal affects appetite and digestion.
