A 2025 review published in the journal Foods synthesized evidence across in vitro, animal, and human studies and found olive-derived polyphenols consistently enhance beneficial bacteria including Lactobacillus and Bifidobacterium, reduce potentially harmful bacterial groups, and increase short-chain fatty acid production while strengthening the intestinal barrier.[1] The review also found evidence of a gut-microbiota-brain axis connection, with olive bioactives showing potential to help with neurocognitive outcomes through this gut-first pathway.

A finding that goes beyond genus-level categories

Most gut bacteria research stops at the genus level, like "more Lactobacillus" or "less Bacteroides." A study looking specifically at strain-level changes found that mice fed a diet enriched with extra virgin olive oil developed distinct Enterococcus strains compared to mice on other diets, and these EVOO-associated strains showed less antibiotic resistance and a more favorable overall profile.[2] This is a more precise finding than most food-and-gut-bacteria research offers, showing olive oil polyphenols shape which specific strains within a bacterial genus thrive, not just the genus population size.

A study comparing how healthy young and elderly individuals' gut bacteria break down olive polyphenols found that age itself affects the biotransformation process, meaning the same amount of olive oil polyphenols may produce a somewhat different set of metabolites depending on how old you are and what your existing gut bacteria composition looks like.[3] This is a useful reminder that gut bacteria research findings often need to be read as "on average" rather than identical for everyone.

What real human trials are testing right now

Beyond lab and animal work, an ongoing clinical study in Portugal is specifically evaluating a polyphenol-rich local olive oil's effect on gut microbiota composition alongside blood and urinary biomarkers.[4] A separate completed trial found EVOO consumption improved inflammatory biomarkers and modulated both gut and saliva microbiota composition, extending the research beyond the gut alone.
