Keto flu: what causes it and how long it lasts

Keto flu hits in the first few days of a sharp carbohydrate cut, bringing headache, fatigue, dizziness, nausea, brain fog, and muscle cramps that resemble the early stages of a viral illness. The cause has nothing to do with a virus and everything to do with water, sodium, and how the body switches fuel sources.[1]

A second factor compounds the electrolyte shifts. Cutting carbs sharply also cuts the fiber that came from grains, fruit, and starchy vegetables, which can disrupt normal gut function and bring on constipation or bloating on top of the fatigue.[1] For most people, symptoms peak within the first few days and settle within one to two weeks as the body adapts to running more on fat and ketones.

1. Hydrate deliberately. Increase water intake and keep sodium intake adequate, since both are being lost faster than usual; plain water covers this better than a sports drink for most people during this adjustment.
2. Eat enough. Under-eating during the transition compounds fatigue; adequate calories support a smoother adjustment.
3. Ease in gradually. A gradual carb reduction, rather than an abrupt cut, gives the body time to adjust fluid and electrolyte balance.
4. Scale back workouts. Lower-intensity exercise in the first week or two prevents symptoms from compounding.

What to do with this

If keto flu symptoms haven't improved within two weeks, that's outside the typical adaptation window, so check with a doctor rather than assuming it will resolve on its own. Adding a source of sodium, potassium, and magnesium in the first week, through broth, low-carb vegetables, or an electrolyte supplement, addresses the biggest driver of these symptoms directly. A gradual transition over one to two weeks, rather than cutting carbs to near zero overnight, tends to produce a milder adjustment period.
