Keto and your gut: what the research shows

The ketogenic diet, very low carb, moderate protein, high fat, has genuine medical roots. It was developed a century ago for childhood epilepsy, and now has a large modern following for weight loss and metabolic health. Its relationship with gut bacteria is one of the more thoroughly studied and mixed pictures in this diet series.

A real human crossover trial

Researchers had a group of people follow a standard diet for four weeks, then switch to keto for another four weeks, testing everyone's stool the whole way through.[1] Once people switched to keto, the makeup of their major gut bacteria groups changed markedly, and one shift showed up again and again: a real, repeatable drop in Bifidobacteria, a type generally considered beneficial.

Why the drop happens

The most direct explanation comes down to fiber. Bifidobacteria rely heavily on fermentable fiber from carb-rich plant foods, and cutting carbs as strictly as keto requires makes it hard to hit the fiber intake your gut bacteria want.

An honest complication: ketones themselves may help

One wrinkle complicates the simple keto-is-bad-for-your-gut story. The ketone bodies your body produces on this diet seem to directly affect gut bacteria, and in mouse studies, that effect showed up as lower inflammation, separate from anything to do with fiber.

What longer-term data shows

Kids with epilepsy following the diet for three months ended up with lower Bifidobacterium and higher E. coli, though their overall bacterial variety held roughly steady.[2] Adults who stuck with keto for six months saw their gut bacteria variety decline over that stretch.

What to do with this

If you're following or considering keto, lean hard into the lowest-carb, highest-fiber vegetables you can find, leafy greens, broccoli, cauliflower, zucchini. That matters more on this diet than almost any other. Add a fermented food daily too, since live bacteria can help offset some of that beneficial-bacteria decline even when fiber is scarce. If keto's restrictions feel too strict, a gentler low-carb approach may be easier on your gut bacteria than keto's extremes, similar to what we found looking at the carnivore diet. If you're on keto for a specific medical reason, loop your care team in on gut monitoring rather than adjusting things solo.
